Game Sheets (Chatsworth Soccer Club)

U12 thru to U18 - Lakeshore Soccer League - Electronic game sheets will be available for each game.  If the Electronic game sheet is not working, it is expected that a paper copy of the game sheet will be completed and submitted to the convener. 

1. Each coach will go to the electronic game sheet site (link can be found on the Lakeshore Soccer League website)
2. Enter in the game number (found on the schedule)
3. Enter in the information for your team (coaches and players present for the game)
4. Submit the form
5. Once both coaches have entered in their information, the referee will go to the electronic game sheet site and enter in the game number
6. The site will indicate if one, both, or neither of the coaches have completed their portion of the form
7. The referee will enter in their information
8. After the game, the referee will enter in the goals and cards
9. Once the referee has completed the form, they will submit it (this locks the form and sends it in)
10. An email with the game sheet information will be sent to each coach and the referee
